Thank you for your interest in serving at Park West Church! In order to more quickly place you in an area of service, please complete our Ministry Application Questionnaire.

There are TWO ways to complete the Questionnaire:

  1. Print a Paper Copy

  2. Submit the Questionnaire Online
    1. Fill out the online Ministry Application Questionnaire and submit immediately. Click here to complete the form now.
    2. Please follow the instructions and complete the Questionnaire to the best of your ability. Your application will be reviewed and your information will be given to the appropriate leader(s) who will be contacting you as soon as possible.
